All events of birth shall be registered in the Register of Births within 30 days. Such event shall be reported by the Hospital/Clinic Administrator, the Attendant at Birth or the Parents themselves. If not registered within 30 days, it is considered delay.
Steps in Securing Birth Certificates (Transciption Form/True Copy) 1. Proceed to the Office of the Municipal Civil Registrar. 2. Supply the following information: a. Your complete registered name b. Date of Birth c. Name of Parents 3. Proceed to the Treasury Office for the payment of fees. (P60 for local & P110 for abroad) 4. Present your official receipt to the Office of the Municipal Civil Registrar for the processing of your request. Requirements for Delayed Registration of Birth Original NSO Birth Certification (No Record on File) Any two of the following documents to show name, date and place of birth, and parents of the registrant:
- Original Baptismal Certificate
- Certified Copy of Form 137/School Records
- Certified copy of Voter Registration Record, if a voter
- Employment Service Record, if employed
- Certificate of Marriage, if married
- Certified copy of the registrant’s Medical Record
- Income tax return of parents or registrant
- Insurance policy, if insured
Duly accomplished Certificate of Live Birth Duly accomplished Affidavit of Delayed Registration Duly accomplished Affidavit of Acknowledgment/Admission of Paternity, if the child is illegitimate Duly accomplished Affidavit of two disinterested persons who might have witnessed or known the child’s birth Note: Present Originals with three (3) clear copies
Requirements for Legitimation by Subsequent Marriage If child has not yet been acknowledged, execute Affidavit of Paternity/Acknowledgment. Secure certified copy of Affidavit of Paternity Execute Affidavit of Legitimation Certified copy of Birth Certificate Certified copy of Marriage Certificate Community Tax Certificates of Parents
Note: If child was conceived and born from parents with prior existing marriage or when parents base legal impediment to marry, registration of legitimation cannot be acted upon.
