- All events of death shall be registered in the Register of Deaths within 30 days from the time of death
- Events of death shall be reported by the nearest relative or person who has knowledge of it
- If not registered within the prescribed period, it is considered delay
Steps in Securing Death Certificates (Transciption Form/True Copy) - Proceed to the Office of the Municipal Civil Registrar.
- Supply the following information:
- Complete Name of the Deceased
- Date of Death
- Proceed to the Treasury Office for the payment of fees. (P60 for local & P110 for abroad)
- Present your official receipt to the Office of the Municipal Civil Registrar for the processing of your request.
Requirements for Delayed Registration of Death - Original NSO Death Certification (No Record on File)
- Four (4) copies of Certificate of Death which must be accomplished correctly and completely
- Affidavit for delayed registration (at the back of the Certificate of Death) executed by the hospital/clinic administrator if the person died in the hospital, clinic or similar institution; or, by the attendant-at-death if the person died elsewhere. In the default of the hospital/clinic administrator or attendant-at-death, the affidavit shall be executed by any of the nearest relative of the deceased, or by any person having legal charge of the deceased when he was still alive
- Authenticated coy of the certificate of burial, cremation or other means of corpse disposal.
